It may appear redundant to ask for the Lawyer since the slip already requests the attorney firm. Usually, the attorney firm is identical to the lawyer in cases where firms consist of a single attorney or the firm is incorporated under the one name. The Lawyer line is for the benefit of specifying which attorney in particular belongs to the case to which the run slip you are filling out belongs.

For Date, you can use any format whether it involves spelling out the month, day, and year or involves only stating the numerical representation (i.e. August 17, 2001 or 8/17/01). The AM/PM feature is to be circled or underlined so that when the request is made (the time when the run slip is submitted to Legal Express) there is an indication as to what time of the day the request was submitted.

Case# slots do not always apply, but in the case where legal documents are being handled it is paramount that a case number be supplied (i.e. A123456, BK-12345-001-ABC, D234567, etc . . .)

The Case Name field serves to underscore the case number. There are situations where executing a task requires a name (e.g. traffic violation matters) either in addition to or in the place of the case number. The case name should be succint and readable (e.g. Widgets, Inc. v. John Doe, et al.).

The Name of Document line benefits everyone. 90% of questions placed to Legal Express about document filings can be answered simply by acknowledging the document's caption. You should include as much information as possible, even if the document is not a pleading (i.e. Letter to John Doe, Esq.). In matters pertaining to legal documents, include the important elements of the name (i.e. Motion to Withdraw as Attorney of Record and Order Shortening Time). Often, abbreviations can be used (i.e. Motion to W/D as Attorney & OST).

We understand that nobody likes to write documents twice, but including as much information as possible on run slips insures accurate information and quick responses to queries.
